Campus Medical Home

Our proposed student “medical home” will provide students with their primary care medical needs, and will coordinate services for which they cannot receive at the college, for example, emergency care and specialty care. Our focus is to promote personal health and academic success, prevent health problems, and to heal. Timely, convenient, and confidential access to medical services is a critical component of delivering quality clinical, counseling, and prevention services. We will also promote proactive health measures amongst students through health education programs and services.

Team Approach

A “medical home” is a team approach focused on providing comprehensive, continuous, accessible, compassionate, and culturally-competent care to patients. Through this approach, HPL understand the importance of quality, coordinated medical, mental, and oral healthcare. We embrace the medical home approach as a means of ensuring health maintenance and preventive care, health education, acute and chronic illness care, coordination of medications, specialists and therapies, and hospital care. Through our “medical home” approach, we provide comprehensive care that includes the student, healthcare professionals, and mental health providers working together to identify and provide all medical and non-medical services to help the student.

Working with Community Providers

In addition to treating their medical needs onsite at the campus health center, students will be linked to appropriate educational and community-based services that will assist in administering the treatment plan. Our providers will communicate and collaborate with all service agencies and will maintain a centralized record of relevant information pertinent to the patient. As a part of the “medical home” approach, we will provide same-day appointments, will always involve the student in all decisions, and provide information on the student’s health.
